After rewatching ? last night, I came to the conclusion that the incident mentioned in the swan orientation film is not a reference to the events seen in the episodes incident part 1 & 2.
The video is copyright 1980 and is designed to monitor other stations, including the Swan, which means it was build after the other stations it has been designed to monitor.
Chang also still has his arm in this video. I think his arm injury at the swan site was meant to make us thing that's where he loses it.
There will be another incident which will occur inside the swan and will likely be the result of someone using the computer to communicate with the outside world. It will take place "not long after the experiments begin.
