- May not be Kate's biological father. Kate was never told that Wayne is her biological father. She jumped to this conclusion after finding out that Sam Austen could not be her father, as he was in Korea at the time of her conception.
- At the point he re-enters Kate's life at the age of 5. He may have very well just left Juliet Carlson's household.
If Mr. Carlson leaves Juliet's family during the divorce when she is around 12 or so. There is a possibility (given the strong younger-version resemblence)that the following scene would then reflect not only the current quadrangle, but possibly also, the the one of the past. Diane Janssen, Wayne, Sam Austen and Juliet Burke Mom.
MOTHER: Yes. But... we need you both to understand that your father and I... we still love each other. Just because two people love each other, doesn't always mean they're supposed to be together.
JULIET: But what if you are supposed to be together?
MOTHER: We're not, honey.
JULIET: But how can you know for sure?
MOTHER: We just know.

AUSTEN: I didn't tell you because I knew you'd kill him. And your mother loved him. You were 5 years old. I wanted to take you along with me. She wouldn't let me.
The age differences between Juliet and Kate would pan out, with his leaving one and arriving at the other.

Wayne and the Spirit Whispers[]
I was recently rewatching the second season, and in the episode "What Kate Did" Kate believes that Sawyer was possessed by the ghost of Wayne Janssen after Sawyer jumped up and asked her "Why did you kill me?".
If you believe what Michael says in the most recent episode, that the Whispers are the people's spirits trapped on the island for what they have done in there lives, then it could be possible that Wayne's spirit is trapped there and possessed Sawyer in his near death state.
